By registering online I hereby grant permission for my son/daughter to attend the Mike Lee Basketball Player Development Academy and authorize the use of any photographs, video or audio of my dependent captured at the Academy to be used in the promotion of future Mike Lee Basketball events. I understand that the Academy will be extremely intense, physically demanding, and potentially hazardous to the health of my dependent. I also grant permission to Mike Lee Basketball to act for me according to their best judgment in any emergency requiring medical attention and agree to hold harmless and indemnify Mike Lee Basketball, Saint Eugene's Grade School, the Madison Area Technical College, the University of Wisconsin-Wood County/Marshfield, the Marshfield Area Catholics Schools their offices, agents, independent contractors, and employees, from any and all liability, loss, damage, costs, or expenses which are sustained, incurred, or required arising out of the actions of my dependent in the course of the camp.
DO NOT REGISTER UNLESS YOU HAVE BEEN INVITED OR HAVE APPLIED AND BEEN ACCEPTED INTO THE ACADEMY.
NO REFUNDS WILL BE ISSUED.
